Customer Identification Requirements

According to the USA Patriot Act we are required to help the government fight the funding of terrorism and money laundering activities. Federal law requires all financial institutions to obtain, verify, and record information for each person who opens a mortgage account.

What this means for you: When you apply for a mortgage, we will ask for your name, address, date of birth, and any other information that will allow us to identify you. We may also ask to see your driver’s license or other identifying documents.

  3. Download a full residential mortgage application* that can be printed out and faxed or mailed back to us. If you choose this option, please also download our credit release form*.
